Best Place to Try It
A ship deck is the best place to attempt this trophy.
You want a group of enemies standing close enough together, with at least one enemy near a railing.
That gives you room to drop the smoke bomb, use the blowpipe, and then knock an enemy into the railing for the final part of the sequence.
Start With a Smoke Bomb

Begin by getting close to a small group of enemies near the railing.
Press Down on the D-pad to drop a smoke bomb. This stuns the enemies and gives you a safer setup for the next step.
Try to stay close after dropping the smoke bomb so you can continue the sequence without losing the group.
Shoot a Berserk Dart

Next, use your blowpipe and make sure berserk darts are selected, not sleep darts.
Hold L2 and switch dart types if needed, then shoot a berserk dart into one of the enemies inside the smoke.
Using the wrong dart type can ruin the attempt, so double-check before firing.
Kick the Enemy Into the Railing

After the smoke bomb and berserk dart, move into position beside an enemy near the railing.
Use R2 + Square to kick the enemy into the railing, then follow through with the takedown.
