Find Gaz’s dog tags in Escape, open the safe using code 8410, and unlock the Never Forgotten achievement in Modern Warfare 4.
Gaz's Dog Tags are a hidden collectible in Escape (Mission 13) of the Modern Warfare 4 campaign. Recovering them unlocks Never Forgotten, a Silver trophy worth 50 Gamerscore.
The answer to the puzzle is simple: the safe code is 8410. The tricky part is knowing when to look for the safe, because it appears later in the mission—not in the initial prison cellblock.
1. Where to Find Gaz's Dog Tags
The dog tags are kept in a wall safe inside an office after the barracks combat section. During the escape from the holding facility, continue through the stealth portion and then the fighting alongside Ghost. When the route takes you through the barracks and into an office, check the wall for the safe's keypad before following the objective onward.

2. Escape Safe Combination — 8410
Interact with the office safe and enter 8410. Open it and pick up Gaz's Dog Tags. Simply opening the safe is not enough: make sure the item is collected.
You should see the Never Forgotten achievement notification after taking the tags. No extra combat challenge is required for this trophy.
3. How to Find the Code Normally
Escape contains two notes that explain the hidden safe:

Reading these notes helps explain why the tags are stored there and contributes to the campaign's separate Noteworthy challenge, which requires 50 different notes.
If you already know the combination, you can enter 8410 without collecting the clue notes first. If you are working toward 100% completion, read both while you are nearby.
4. What If Never Forgotten Does Not Unlock?
First, check that you picked up the actual dog tags rather than only unlocking the safe door. If the item is gone but the notification did not appear, check your platform's achievement list and try replaying the mission if necessary.
The dog tags are a collectible, not an automatic reward for completing Escape. If you go past the office without opening the safe, the mission can continue without awarding this achievement.
